SUMMARY: The Special Populations Outreach Manager will identify and conduct strategic support to drive outcomes for benefit enrollment and referral services for eligible central and eastern Ohio residents. A specific focus is on developing and managing partnerships that provide customers with resources and benefits to support Social Determinants of Health. Key programs for enrollment will include, but not be limited to, SNAP, Medicaid, WIC and HEAP. Special populations served will include individuals preparing for their release from incarceration; seniors; and students, as well as other identified groups in need of special attention.


  • Identify "special populations" who may or may not be engaged in food and nutrition services provided by MOFC to provide SNAP application assistance.
  • Distribute materials about SNAP benefits, eligibility, and the application process. Pre-screens prospective applicants to access likelihood of eligibility.
  • Using Ohio Benefits Self Service Portal to pre-screen and assist interested members with enrollment into eligible benefits such SNAP, Medicaid, OWF.
  • Provide Re-entry Clinics to improve transitioning inmate's accessibility to SNAP Benefits
  • Oversee coordination with Ohio Department of Rehabilitation and Corrections and work collaboratively to develop and implement a system that efficiently supports Re-entry Outreach
  • Oversee handling of client personal documents and ensure confidentiality of information. Assure all personal information collected and reviewed through the Outreach process is handled in accordance with privacy statements and policies.
  • Submit internal and external programmatic reports to internal and external stakeholders that summarize the status of the services provided and customers served.
  • Work collaboratively with identified partners to arrange customer referral and service provision as needed for special population
  • Coordinate resources events, presentations, public speaking engagements and other initiatives targeted to special populations.
  • Travels to and within service area to reach residents as necessary.


Education and Certifications: Bachelor's Degree in related Social/Human Services field preferred or 5-10 years in related outreach or social services field. Must have a High School Diploma/GED and valid Driver's License.

Experience: At least 3 years of social service/human services experience or work in non-profit environment. Strong communication skills and experience with conflict resolution. Knowledge of and experience with public benefits and community resources preferred, specifically SNAP, Medicaid, Ohio Works First, Women Infants and Children, CSFP, and the Home Energy Assistance Program.

Skills/Competencies: Ability to interact with diverse communities and personalities. Ability to listen, empathize, and strategize while maintaining healthy boundaries. Candidates must represent Mid-Ohio Food Collective with professionalism when interacting with clients, volunteers, community members and Food Collective staff. Dual language a plus.


  1. Working Conditions: Will perform duties from field; frequent travel within assigned service area. Reliable transportation is required. Applicants must have received or be willing to receive the COVID-19 vaccine by date of hire to be considered for employment
